Examination of teeth, gums, all soft tissues, extent of movement of jaws, and TMJ (Temporo-Mandibular Joint) reveals that he is in good health. Her medical and dental history indicated that he had no problems or issues contraindicating  treatment for her compulsive overeating using OJW.  A routine panoramic X-ray was taken, which further showed that his and TM joint were normal. His occlusion was tested with articulating paper in order to test for/locate any possible traumatic occlusion. None was noted. (The Temporo-Mandibular Joint (TMJ)) is a "ball and socket" joint. The "ball" is the top part of your lower jaw and is called the "condyle". You can feel them by putting your fingers just in front of your ears when opening and closing your mouth. She was advised that in OJW the jaws are wired together but the teeth of the upper and lower jaw touch just barely, thus allowing about 1.0 mm of lower jaw movement in every direction. Consequently, no pressure is placed on the TM joint. She signed the informed consent for jaw wiring . She was provide an overview of the entire procedure and was advised of the dangers he might encounter and how to avoid them. The informed consent and the FAQ's were reviewed and he signed the OJW Telephone memo indicating that he was aware of all aspects of the OJW treatment method for the control of compulsive overeating. "Begg" brackets were bonded on upper teeth 4,5,6 and 11, 12, 13 and lower teeth 29,28,27 and 22,21, and 20 (canines and first and second premolars). Tooth 3,4,5,and 6  porcelain faces and were specially prepared to assure good bracket-tooth adhesion and the others were acid etched as they usually are for bonding brackets. The brackets were micro etched. The brackets were "double bonded", in that a second layer of adhesive was place on the flanges of the bracket. All excessive and unsightly adhesives was removed and the space between the teeth was checked for unimpeded passage of dental floss. The bracket slot was tested for blockage that might have occurred from stray adhesive. Finally, he was asked to put His teeth gently together and the wiring was done with "dead soft" .014 inch diameter stainless steel round wire.  She observed the wiring technique and then cut out the wire as a practice procedure which he did with ease in about 5 seconds. He was then wired closed on the right and left sides so that he felt equally comfortable on both sides with about 1.0 mm of freedom to move in all directions. He agreed to follow the recommended protocol in His case a liquid diet of not more than 1850 calories each day. He had done His own research on the number of calories that he would require to lose weight at about the rate of 1.75-2 pounds /week: After 5 weeks of being wired he would remove the wires for 5 days to exercise His TM joints. He was advised not to rewire if there was still any stiffness until the TM stiffness disappeared.  He was given a kit of supplies including a wire cutter and a wire twisting instrument. (Mathieu needle holder) He was given instructions on how to exercise His jaws  during the 5 days he would be released from the wires and advised re the possibility of the joint becoming more limited in motion over time (two fingers of opening rather than 3 fingers).  He was counseled to adhere to a 1850 calorie  low/no salt liquid diet. This figure is 500 calories higher than usual because he was deemed to be a large-boned man (6 feet tall). He was advised to always carry the wire-cutter with him. Lacking a wire cutter a simple fork could accomplish the emergency removal of the wire. He was provided with instruments to assist in the removal and replacement of the wires including and (Mathieu needle holder). knowing that he would not be returning every five weeks to be rewired my staff and I taught him how to rewire himself, a task he was able to accomplish with remarkable ease. The exact  sequence of wiring is shown at OJW jaw wiring sequence. Herein I advise him to weigh himself EVERYDAY and even better keep a graph of His weight loss 18"wide (each day) by 9" high (weight) and make an entry every day/week.  I encourage you both to read:
the letter A.T. wrote  who spells out for you what her experience was like warts and all.

SPECIAL NOTE:

Ps. Marco and his sister arrived on the day we had 7 inches of snow and was on his flight back to California 3 hours after I placed the wires on him and his sister.  It gave me great pleasure to note that Marco and Nancy were most pleased with service we provided them at this office. Even more surprising was that they chose to come to NY since I had told them about two other providers available to them: one in  Hollywood CA and one in Littleton, Colorado. Best wishes to both of you in this endeavor. When I receive the photos you took I will post them :-).

Dear Dr Ted,
I'm so sorry for not getting these to you sooner or at least contacting you and your great staff. Let me please start by telling you how impressed we are with your staff. Christina is a sweetheart and quite a professional. (sure doesn't hurt that she's beautiful) From my first contact until we left, she didn't miss a thing. Your assistant Brian was equally impressive, always staying ahead of you and anticipating your every move. He made sure we had everything ready for our trip home and made sure we understood it. Great people are hard to find. Congratulations! Finally the beloved Dr Ted. You rarely find someone you immediately bond with and trust. From the first time I called you I knew I wanted you to do the procedure. Although on your advice I contacted the other 2 providers allot closer, (one right here in So Cal) and did not feel the love. Our trip across the country was worth it. I can not thank you enough for all your help, guidance, and advice, thus far. I hope your profession gives you the recognition you deserve. Mavericks and trailblazers don't often get the kudos afforded to those who come after. I believe you will be known as the grandfather of the OJW for weight loss. As you know, my attempts to wire my own mouth shut ended in failure. But thanks to you and your technique, I believe it to be far superior for comfort and control.  I will try to bring this to the masses on the west coast in the hopes of helping people like me and your father who had given up on being "normal". Doctor, I shall never forget your kind words, teaching spirit, gentle touch, great staff and happy soul. I look forward to working with you in the future, but mostly to calling you friend. Please let me know what to do to start writing on your web site.
With Great Sincerity,
Marco Suarez
P.S.     I've already lost 15 pounds!
