Dear Doctor: 

I will be fabricating a removable vacuum formed orthodontic appliance for this patient.

Please make an alginate impression of the upper and lower dentition and pour them up in stone not plaster. Plaster is too soft and is not an acceptable medium for transportation of the molds to my office where I will be fabricating  a vacuum formed appliance for the patient. Also make a  duplicate set of these molds.

Please take the utmost precaution to pour up the impressions without delay so as to avoid any distortions in the molds produced. As you well know the smallest void especially at the cusp tips and incisal edges of the front teeth will render the impression inaccurate and therefore useless. The duplicate impressions should be poured immediately after removing the primary molds from the original impression.  The patient is counting on you to prepare accurate dental molds which they will be shipping to my office.  As you well know the fit of vacuum formed appliances demands accurate dental molds.

If you know the patient and believe that dental work is imminently needed, (dental work that will make a full coverage vacuum formed retainer non-fitting) please advise them. Please feel free to call me if it will benefit the patient or clarify any questions you may have.  The patient and I are counting on you and your staff for accurate dental models.  

Finally, I have instructed the patient to mail the models to me so please give the models to them.

Ps.  To the patient:

1. The taking of an impression and then pouring up of a mold of your teeth is typically done by the dentist's assistant.  If you would like Dr. Ted to contact the dentist of your choosing and let him/her know that you are coming in for an impression, Dr.Ted will be happy to do that if you provide the dentist's name and telephone number.  2.  If you are making a BrB for the upper teeth alone you can save some money by just having the dentist make an upper mold alone and not duplicate it if the charge seems excessive. But remember, the mold is fragile and can break accidentally either before they reach me or after they reach me (before the BrB is made). If that happens then I will have to ask you to have another made. Hence, maybe a duplicate is the best course of action. 3. Do call the office first to ask the cost of making an upper mold of your teeth. My guess is the fee will vary between $25 and $65.
