If/when you decide you want to order a Brb follow the instructions below exactly.

First read and understand the Informed Consent document. [see Informed Consent document]

Sign and Date it, and indicate how you were referred to me and mail it to: Dr. Ted Rothstein, 35 Remsen St., Brooklyn, NY 11201 ALONG WITH:

Your choice of BrBs [see choice of BrBs] AND ALONG WITH

Your dental molds: download the prescription/order to the dentist [see prescription/order to the dentist]. and have him/er make the necessary dental molds of your teeth and give them to you (see below).  Mail all the above ALONG WITH a payment (see below) equal to the sum of the BrB item(s) you want to order AND ALONG WITH any special requests for color-ties (see below) ALONG WITH the address to which you want the item sent.

Your order will be completely ignored/rejected if any part of the above items are missing:

1. Two identical molds for each BrB you want. (Ex. You want one upper BrB then you sent two duplicate upper molds (NOT one upper and one lower) 2^nd Ex. You order one upper and one lower BrB then you send two duplicate uppers and two duplicate lowers. [See the prescription you can give to the dentist whom you choose to make them for you.]

2. An Informed Consent document wherein you have signed your first and last name and dated and indicated how you were referred to me.

3. A money order or CERTIFIED check (no personal checks accepted) for the amount of the item you are purchasing. You may call the office to phone in a credit card payment at 718 852 1551. Please tell the staff you are calling to make payment for a BrB or simply put... the "Fake braces"

4. The specific BrB(s) you want made and if colored elastic attachments are or are not desired. (see below)

5. The Recipient’s name (If other than financially responsible party’s name) Name and Address to which you want the BrB sent.

AND DON'T FORGET:

Orders that are missing vital parts will be held for thirty days and then discarded.

If a BrB orthodontic appliance is ill-fitting (fits on the molds but not on the teeth) you will have two choices: 1. To ask for a refund of 50% of the amount you or 2. pay 50% of the amount you paid to have the BrB remade again on new models that you will need to supply. In either case you must return the non-fitting BrB.

We allow at least 3 days to pass before fabrication of your BrB begins. Once fabrication is under way no refunds will be considered.

All arch wires are ligated/tied with .009 ligature WIRE only on the four front teeth unless you ask for more. SILVER COLORED Elastic ligatures are placed on all other teeth.

You may specify color elastics at an additional charge. [Show me the selection of colors]
