DDS system has a professional locator page that I used to find the providers of the DDS appliance closest to my office in Downtown Brooklyn. Lo and behold I found at least 6 providers who I know well. So I called a few to say hello. A few of them had already provided the appliance to some of their patients. When I asked them about the Informed Consent for the service they said they felt it was an excellent document and that it clearly spelled out the extent of the appliance's ability. You will this fine soft-spoken doctor with a slight Southern drawl just the right guy to provide the DDS appliance to you. Go see him. (Number 1 on the list.)
