These forms may be copied, completed and filed by mail. The NYC Small Claims Courts are listed below with addresses and telephone numbers:
WARNING IF YOU FILE BY MAIL
It will not be accepted if you have used the wrong form i.e. using a "business" (commercial claims part statement of claim) form when you should have used an "individual" (small claims part statement of claim) form.
If you have not completely and correctly filled out the form
If you have not provided the first and last name of the person you are suing and a proper address (no P.O. box numbers)
If you have not paid the correct filing fee:
Individual claim from $1 to $1,000: filing fee $15
Individual claim from more than $1,000 to $5,000: filing fee is $20
Commercial claim from ($1.00-$5,000.00) all the same fee $30.62
If you have not made your payment by certified bank check or money order
If you have not included a stamped self-addressed envelope so that you may receive back your case index number and court date.
