However, the forms in the book are specific for the residents of NYC, New York State, Connecticut and New Jersey.The forms shown and provided here are the actual forms used by the Small Claims Court Part of the Civil Court of the City of New York.

These forms are accepted by each of the Small Claims Courts of the five boroughs (counties) of NYC (Brooklyn (Kings County), the Bronx, Manhattan, Queens, and Staten Island (Richmond County).
